What is eMR?
Electronic medicine reconciliation (eMR) involves using computers to ensure the right medication is given to the right patient at the right dose and at the right time. It is a process that identifies the most accurate list of a patient’s current medicines – including name, dose, frequency, and route – and comparing them to the current list in use, recognising any discrepancies, and documenting any changes.
